- In the last 14 days, you have been in close contact with someone who has a fever, sore throat, cough, chills, shaking chills, muscle or body aches, loss of taste or smell, vomiting or diarrhea, difficulty breathing, or shortness of breath.
- In the last 14 days, you have tested positive or been in contact with someone who has a confirmed diagnosis of COVID-19.
- You have been tested for COVID-19 and are awaiting the test results.

- Please bring a picture ID and your medical insurance card(s) to each appointment.
- If it has been 6 months or longer since you were last seen at VRA, please bring a list of your current medications.
